Required Documents for DUI Evaluation Appointment

    Court purpose drivers abstract that can be obtained at your local DMV or online at ilsos.gov

  • 2.

    Notice of Summary Suspension or Law Enforcement sworn report issued to you by the arresting officer. This may also be obtained from your attorney or the courthouse.

DUI Evaluation and Classifications

The DUI evaluation determines your DUI risk classification. There are four different levels of DUI offenders: Level I Minimal Risk, Level II Moderate Risk, Level II Significant Risk, and Level III High Risk. This classification determines the number of hours and level of care you will need to complete. We specialize in working with the Illinois Secretary of State. Groups are offered five days a week, including weekends. Flexible scheduling is available.
